Project Hope

The HOPE Centre is a community based non communicable disease project, focused initially on providing comprehensive diabetes services to Zandspruit – a township on the outskirts of Johannesburg as well as three other neighbouring townships that fall within out catchment area.

The program has three components:

  1. Increased PREVENTION activities through building th e capacity of existing community health workers by training them in NCDs with a focus on diabetes and piloting new screening and monitoring tools to ensure successful referrals.
  2. Increased access to DIAGNOSTIC and TREATMENT services in the community by running a diabetes clinic in partnership with the government clinic, providing a new range of diagnostic and treatment services that are currently not available.
  3. Improved diabetes MANAGEMENT through the establishment of peer support groups that will provide health education, nutritional support and exercise support to help improve longer-term health outcomes.
