The HOPE Centre is a community based non communicable disease project, focused initially on providing comprehensive diabetes services to Zandspruit – a township on the outskirts of Johannesburg as well as three other neighbouring townships that fall within out catchment area.
The program has three components:
- Increased PREVENTION activities through building th e capacity of existing community health workers by training them in NCDs with a focus on diabetes and piloting new screening and monitoring tools to ensure successful referrals.
- Increased access to DIAGNOSTIC and TREATMENT services in the community by running a diabetes clinic in partnership with the government clinic, providing a new range of diagnostic and treatment services that are currently not available.
- Improved diabetes MANAGEMENT through the establishment of peer support groups that will provide health education, nutritional support and exercise support to help improve longer-term health outcomes.
